Patient Forms

If you are a new patient to our office, please visit our patient forms page to complete our new patient bundle with forms that will need to be filled out when you arrive at our office. Filling them out will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance

Our office accepts accept all PPO insurance plans. We are currently In-Network providers with Cigna, United Concordia Elite, and Aetna Extend plans. Please contact our office with your insurance details to learn more.

We also offer an in-office Discounted plan for our Cash patients. Please call our office to discuss our Optimal Smile Club Program or visit our Optimal Smile Club page.
